Requirements for Seaman's Passport

1. Guarantor’s Details
   1. Guarantor’s from duly signed by a commissioner of oaths
   2. Photocopy of data page of guarantor’s passport and either
      • Photocopy of National identity card
      • Driver’s license.
2. Birth certificate or age declaration
3. Letter of identification from local government of origin or residence
4. One (1) Passport photograph.
5. Court affidavit and police report in case of lost passport
6. Acknowledgement slip and receipt of payment
